Overview: What CCP Is and How It Works
Coordinated Career Pathways (CCP) is a Regional Center employment service designed to
support adults served by Westside Regional Center who want to work but may be unsure of next
steps.
Through personalized coaching and employer partnerships, CCP helps participants explore
employment options, build real world skills, and move toward meaningful, paid jobs in the
community. The focus is on early planning, clear next steps, and long-term employment success.
Premiere Living Services is vendored under Service Code 956 to deliver CCP in compliance
with Title 17. PLS provides coaching and coordination services only and is not the employer of
record for participants.
For Service Coordinators
Service Coordinators play a central role in CCP. Our team works closely with you to ensure
clarity, consistency, and timely follow through.
PLS partners with Service Coordinators to:
- Receive and confirm authorizations under Service Code 956
- Develop and update the participant’s Person-Centered Career Plan (PCCP)
- Provide clear, documented progress updates at each stage
- Maintain Title 17 documentation standards and transparent communication

For Families and Clients
CCP offers a structured, supportive experience focused on progress, confidence, and
independence.
Participants receive:
- One on one coaching from a Community Employment Specialist (CES)
- Ongoing coordination from a Career Pathways Navigator (CPN)
- Paid, real world work experiences in community settings
Families are included throughout the process, with planning centered on strengths, interests, and
long-term goals.
Outcome Goal:
Support participants in moving toward competitive, integrated employment.
For Employers
Employers who host CCP participants help create inclusive workplaces while gaining motivated
team members.
Employers can expect:
- The employer or internship site serves as employer of record
- Participants are paid directly by the employer
- PLS provides on-site coaching, integration support, and problem solving
- Ongoing communication and coordination with the Service Coordinator
Hosting a CCP participant supports inclusion while meeting real workforce needs.
