Overview: What CCP Is and How It Works
The Coordinated Career Pathways CCP program is designed to create meaningful, paid work experiences for adults served by Westside Regional Center. Through personalized coaching and employer partnerships, CCP helps each participant gain independence, confidence, and long term career readiness.
Premiere Living Services is vendored under Service Code 956 to deliver job coaching and related employment supports in compliance with Title 17. Our focus is on individualized pathways that match each person’s skills, interests, and IPP goals. PLS provides coaching and coordination services only. PLS is not the employer of record for participants.
For Service Coordinators
Service Coordinators are central to CCP success. Our team works closely with you to:
• Receive and confirm authorizations under Service Code 956.
• Develop and update the participant’s Person Centered Career Plan PCCP.
• Provide clear, documented progress updates for each phase of the internship or employment experience.
• Maintain Title 17 documentation standards and transparent communication throughout.

For Families and Clients
Families can expect a structured, supportive experience focused on progress and independence.
• Each participant receives one on one coaching from a Community Employment Specialist CES.
• A Career Pathways Navigator CPN coordinates services, communicates with families and Service Coordinators, and keeps goals on track.
• Clients gain practical, paid work experience in real community workplaces, developing confidence, teamwork, and self advocacy.
Outcome Goal: Help participants transition from supported work into ongoing competitive integrated employment.
For Employers
Employers who host CCP participants play a vital role in building inclusive workplaces.
• Your organization employs and pays the participant.
• PLS provides on site coaching, workplace integration support, and problem solving at no cost to the employer.
• PLS manages documentation, communication with the Service Coordinator, and Title 17 compliance for authorized services.
• CCP staff remain actively engaged to support both the participant and the employer throughout the placement.

What is CCP
CCP stands for Coordinated Career Pathways. It is a Title 17 compliant employment service vendored through Westside Regional Center and operated by Premiere Living Services. PLS provides coaching and coordination services and does not serve as the employer of record for participants.
Who Can Participate
Eligible individuals are active clients of Westside Regional Center with employment goals identified in their IPP.
Are Participants Paid
Yes. Participants are paid by their host employer or internship site for the work they perform. PLS provides job coaching and coordination support through authorized WRC services.
Who is the Employer of Record
The host employer or internship site is the employer of record for the participant. PLS is the employer of record only for PLS staff such as coaches.
What is the difference between a Career Pathways Navigator and a Community Employment Specialist
The Navigator manages program coordination and planning. The Employment Specialist provides on site job coaching and direct support at the workplace.
How are Placements Chosen
Placements are individualized and based on the participant’s skills, interests, and goals with input from the family and Service Coordinator.
How does Compliance Work
All services are provided under Title 17 with full documentation, safety practices, and alignment with the participant’s person centered plan.
